Black Box Thinking
Black Box Thinking, popularised by journalist Matthew Syed, is the practice of systematically investigating failure — treating every significant setback as data to be analysed, not embarrassment to be suppressed. Named after flight data recorders ("black boxes"), it describes a cultural and methodological approach to learning that has made aviation extraordinarily safe. The contrast with healthcare, legal, and business cultures — where failure is hidden rather than investigated — illuminates how much learning most organisations forfeit.
